IBA Health Plans will assist you with any special requirements for restricting uses of your information. Any employer requesting confidential information is required to obtain a written authorization for release signed by the member or dependent that is the subject of the information.
IBA Health Plans includes provisions related to confidentiality in all of its provider contracts, and provider policies are reviewed at the time a provider applies for participation with IBA Health Plans and at each subsequent visit. Members and their dependents have a right to obtain all information related to their records that are in the possession of IBA Health Plans. If you or your dependents are seeking clinical records, you must make a request for those records directly to the provider of those services.
