Disease Management Programs
Disease Management Programs are a proactive, population-based approach to managing care. Programs are available to members with specific conditions who will benefit from education and self-care management support as well as health status surveillance.
IBA's Disease management programs consist of interventions based on nationally accepted medical evidence and designed to help improve the outcome of members with specific chronic disease by improving the course of their disease, preventing acute exacerbation and improving the quality of their lives.
The key components of these programs are to provide education and support to the patient in self-management of the disease, as well as support the physician's treatment plan and monitor compliance.
Candidates for the programs are primarily identified through utilization of health care services and physician referral. IBA utilizes expert specialty providers to work collaboratively with the patient, the physician and the health plan to reinforce the physician's treatment plan, provide education and encourage patient self-management. Programs currently available include Diabetes, Pediatric and Adult Asthma, Coronary Artery Disease, and Chronic Heart Failure.
