The following Protocol is adopted by UnitedHealthcare and its affiliates, including IBA Health Plans. This Protocol supplements the provisions of the Physician, Health Care Professional, Facility and Ancillary Provider Administrative Guide and all other relevent UnitedHealthcare affiliate provider manuals pertaining to laboratory claims and the use of non-participating laboratory services.
