Provider Directory

The directory is divided into sections based on the type of provider you are searching for (see below). You can choose from Primary Physicians, Specialists, Hospitals, Other Facilities, Allied Health, Mental Health or Pharmacies.

Once you have made your selection you will be transferred to a search screen. You will then be able to search by county and specialty, or by physician name. The option labeled "Product Line" refers to your specific health plan. To get the correct listing, you must select the correct product line. To find the correct IBA product line, please reference your IBA ID card. Under the IBA Health Plans logo it will tell you if you are enrolled in the IBA Select or Dual Select Network. If you have not received your ID card, please call our Customer Service department to determine if your provider participates with IBA Select or Dual Select. Product lines include:

• IBA Select - IBA Health and Life Assurance Company
• IBA Dual Select - IBA Health and Life Assurance Company

To search for a provider in your area, please select a section:

• Primary Physicians (Family Practitioners, Internal Medicine, etc.)
• Specialists (Allergists, Cardiologists, Dermatologists, Surgeons, etc.)
• Hospitals (Hospitals, Urgent Care Facilities, Skilled Nursing Facilities, etc.)
• Other Facilities (Anesthesiology, Home Health Agencies, Physical Therapy, etc.)
• Allied Health (Chiropractors, Midwives, etc.)
• Mental Health (Administered through United Behavioral Health)
• Pharmacies (The pharmacy network is maintained by Merck Medco)

IBA Health Plans makes every effort to keep this directory current. However, our network of Participating Providers changes continually. Before you seek services with a listed provider, or if you have questions concerning your physician's participation status, please call the Customer Service number listed on the back of you plan ID card. It is your responsibility to confirm your provider's participation with Customer Service - not with your physicians office. Note: Please make sure you are looking at the correct product line for a participating provider. Not all providers participate with all product lines.
