Ordering Your Prescriptions by Mail
If your medical benefits with IBA or PHP include a pharmacy benefit, you can use the mail service program, as provided in your certificate of coverage, when you need a prescription to treat an ongoing illness or ailment, such as medication to reduce blood pressure.
Completing Your Health, Allergy and Medication Questionnaire
The first time you order medication by mail, complete the Health, Allergy and Medication Questionnaire and return it in the non-window envelope provided. Questionnaires are available through IBA Health Plan's Customer Service department by calling 800.722.3644.
Filling New Prescriptions
The first time the doctor prescribes a medication, ask for two copies of the prescription:
- The first for a one-month supply to be filled at a participating retail pharmacy.
- The second for the maximum supply you need, up to the amount your plan allows, with refills if appropriate. Use this prescription to obtain your medication from the mail service pharmacy.
This will help you start your new medication right away and also take advantage of the savings available through mail service.
Note: Your plan determines the maximum quantity that can be dispensed. The pharmacist's judgment and dispensing restrictions such as quantities allowable govern certain controlled substances (i.e., pain medication, or medication for the treatment of ADD and ADHD) and other prescribed drugs.
Easy Options:
By Fax
Give your doctor your member ID number. Ask your doctor to call 1.888.EASYRX1 (1.888.327.9791) to get instructions on how to fax your prescription to the pharmacy.
By Mail
Complete the Ordering Medications from the Mail Service Pharmacy form. Mail it with your prescription and copayment in the window envelope provided. Order forms are available from our Customer Service department.
Refilling Prescriptions By Mail:
Use the refill and order forms provided with your medication. Mail the forms with your copayment.
To help ensure you do not run out of your prescription:
- Remember to reorder on or after the refill date indicated on your refill slip or medication container. Or,
- Reorder when you have less than 14 days of medication left.

Delivering Your Medication
We promptly deliver your medication by U.S. Mail or UPS. Your package usually arrives within 7-11 days. It will include:
- Medication container(s)
- Instructions for refills
- Information about your medication
Paying for Your Medication
You may pay by check, money order, VISA, MasterCard, Discover/NOVUS, American Express, or Diners Club. If you prefer to pay all orders by credit card, consider joining the automated payment plan. Call 1.800.948.8779. You will be asked to enter your credit card number and expiration date, allowing you to charge future orders.
As a Reminder: Federal law prohibits the return of dispensed prescription medication.
